A C-SNP review should confirm that a plan is available in the client's ZIP/county and that the plan serves the relevant condition category. It should also review doctors, specialists, prescriptions, pharmacies, network rules, and whether the plan's care model is suitable for the client's needs.
A chronic condition by itself does not mean a specific C-SNP is available or appropriate. Eligibility, timing, plan rules, carrier appointment, and provider access must all be checked before any recommendation.
